Exam Request

This form is used to request proctors for classes or to administer make-up examination at the UH System at Sugar Land. It will be sent to the Sugar Land front office for processing.

This form must be submitted at least FIVE working days prior to the exam date.

UHSSL does not accept online exams request and cannot provide proctors for face-to-face courses. Please contact your department for arrangements.

NOTE: All students will be required to show identification to take make-up exams. Unless directed by the professor, all books, notebooks, backpacks and purses will be placed in the front of the classroom during exams or in the UH System at Sugar Land front office for those taking make-up exams.
